COURSE RECORD FORM SELECTION

You are required to keep a copy of this form up-to-date with all pertinent information for each course that you take.
You will need the information on it to fill out latter forms.  It will also be a reminder of where you are in each course.

ATTENTION:  Upon request by SLBC, you will be required to forward copies of your Course Record forms.

Because of failure by many students to follow various procedures, it has become obvious that many are not keeping Course Record forms as required.  Therefore, this requirement of forwarding Course Record forms upon request has been instituted by the college.
  2. If you have not been keeping Course Record forms, and we request that you forward one, you will be required to construct one for the course(s) requested and submit it, or them, before you will be allowed to continue your studies.
  3. How to forward requested copies:
     
If you keep an electronic copy you can attach the file to the Course Record Submission form to send it to us.
      If you keep a hard copy (a printed copy) then you can scan it and send the copy to us attached to the Course Record Submission form.

By using the wpd or the doc formats you will not have to print out a copy of the form for each course that you take.
You can keep a copy for each course in a file on your computer and use those individual copies internally to record information instead of having to print out a copy for each course and record the information on it externally.


TRANSCRIPT and GRADES FORM

It is required for all students to maintain a Transcript and Grades Form and submit it at least twice per year.

Please submit this form to the College on at least a bi-yearly schedule. (Twice per year.)
It is recommended that the form be submitted in January and in June.
If you cannot submit it during those two months, then submit it at 6 month intervals.
A completed form is required from all students before any Certificate or Degree can be issued.
