You must sign this agreement before taking this Closed Book Test in order for your grade to be recorded in your file.

By submitting this form you agree to a legal contract with Salt Lake Bible College and Salt Lake Baptist College.

CONTRACT:
1. By submitting this form you signify that you understand and that you agree that you will take this Test as a "closed book test."
2. By submitting this form you signify that you understand and that you agree that "closed book test" means that you can use no written materials nor any other materials or assistance of any kind while taking this test.
3. By submitting this form you signify that you understand and that you agree that you can NOT be assisted in any manner by another person while taking this test.
4. By submitting this form you signify that you understand and that you agree that should you not fulfill the terms of this contract, such failure to be known by the term "cheating," that you will be expelled from Salt Lake Bible College and/or Salt Lake Baptist College and that your school record will have the notation added, "Expelled for cheating" and that all credits previously earned will be declared null and void.
