Thank you for agreeing to the terms of the Closed Book Test
Contract.  By so doing you have legally agreed that
you will take the Test indicated by you
on the form, as a "closed book" test.

The contract will be placed in your file.

The Password will be sent to you in 1-2 days.
When it arrives, you may take your Test.

If this form was for a Final Test, then before
taking the Final Test, please submit
a "Sign In form" marked "Finish"
if you have not already done so.
