On
behalf of Salt Lake Baptist College I thank you for your
interest in becoming an Extension Campus of SLBC.
NOTE:
When you click on the links below, the pages will open in
a new window; which you can then simply close to return to this page.
PLEASE
READ THIS ENTIRE PAGE BEFORE CONTINUING
1.
Please click HERE
to go to an online application form. Fill out and submit the
online form. If you meet our requirements as stated in the
Extension Campus Agreement form then you will be granted tentative
status.

form can be found in the next section for you to read and/or download.)
2.
After submitting the online application form, then please click HERE
to open a PDF file of the Extension Campus
Agreement form. You may print it out and then complete the form and send the indicated page back to us so
we can review it.
NOTE:
If you are granted "tentative status" through the online
application, then you must
mail us the hard copy so that we have it on file. Upon our receipt
of the hard copy stating your understanding and agreement with all of
the requirements for an Extension Campus, then your status will be changed from "tentative" to
"accepted."
Once
we receive the online form, we will notify you within
several workdays if you have been accepted as an Extension Campus.
As was stated earlier, your status will be marked "tentative" until
receipt of the hard copy in the mail for placement in your file.
