As an affiliate Study Center of Salt Lake Bible College/(hereafter called SLBC) we, the affiliate Study Center, hereby agree to abide by all requirements and operating procedures as listed in the standard Affiliate Study Center Agreement.
Signature of Chancellor or other authorized representative of the school signifies agreement to the terms of these requirements as binding for initial certification and for continued certification of the school as an affiliated Study Center of SLBC.
Initial Application for Affiliate status: Submission of the Application form is for tentative status. You must also mail us the hard copy so that we can have it on file in order for your status to be changed from "tentative" to "approved."
If this is a Renewal: Please indicate that fact on the form. Do not mail us the hard copy. You will be notified if your status has been Renewed for the upcoming school year. School year is September 1st of one year until August 31st of the following year. Renewals must be submitted during the month of September, on or before midnight, September 30th, of the year for which Affiliate is seeking Renewal status.
