INSTRUCTIONS

Password, then DO NOT USE THE FORMS BELOW!
If you were issued a User Name and Password, you are registered with our SLBC
Records site
and you must submit ALL WORK and FORMS through that site ONLY.

you are a student of our Non-Video College, SLBC, and are subject to the ADMINISTRATIVE
FEE,
then be sure that fee is paid up-to-date before submitting your Password
Retrieval form.

FEE, then be sure

As of 7/15/2012 the guidelines were updated and must be followed exactly.

If you have passed all of the Section Tests for a particular course
(if that course has Section Tests) and found
the correct answers to all questions missed on any of the section tests by
restudying the corresponding section of the workbook, textbook, or
your KJV, whichever is appropriate for the questions you missed, and met
the attendance requirements for that course as listed on each of the course main pages, then you
can click on either the
picture or the words, "PASSWORD RETRIEVAL," at the bottom of this page
and request the password to gain entrance to the Final Test for that course.

When you enter your weeks
of attendance for the course, always round down to the lower week.
Example: If you have been in the course for 4 weeks and 6 days, then you
would enter a 4 for your weeks of attendance and not 5 because you have not
yet been attending the course for a full 5 weeks.
For request using the generic Password Retrieval Form, an e-mail will be sent to you, generally within 1 to 2 days,
informing you of the password. This does not include weekends when the
college offices are closed. College offices are closed from 10:00 PM
Friday night until 10:00 AM Monday morning. (Salt Lake City, Utah, USA
time.) If you send your request during the weekend, then do not expect
the password to be sent before Monday afternoon. And if you do not
receive it by Wednesday, then contact us to see whether or not the form
arrived in our email over the weekend. If the Request form submitted properly to us you should receive an autoresponse stating the name of the form you sent acknowledging receipt by us of that form. If you do not receive an autoresponse then the form did not submit properly and we did not receive it. In that case you will have to resubmit the form.
For requests using one of the links below for passwords for a specific test, you will receive the password immediately.
Remember, it is your responsibility to meet all requirements for each course. If you have not met those requirements, then the course cannot be credited to your file.
Should the Password Retrieval form fail to submit properly, there will be instructions on how to submit an emergency email request at the bottom of the form. But please use the form if possible and use the Emergency Email request only when the form will not submit properly.

- NEW PROCEDURE for REQUESTING PASSWORDS -
We are in the process of moving the listings for individual passwords out of the Password Retrieval form and giving links to access them individually under the name of the course in the following list. The requests for passwords for Final Tests will be moved to this list first. Once the course listing for the Final Test Password request is moved to this list it will be removed from the generic Password Retrieval form and you will no longer be able to access it
from that form. If you try to access it from the generic form you will not find it there. It must be requested via the link to it in the following list. All tests, such as Lesson tests, that are not in the following list will still be accessed from the generic Password Retrieval form.
- RESULTS PAGES OF TESTS -
We have experienced some problems with our testing service
provider. Their server has been down several times. To prevent loss
of grades for tests completed we now require that all students make a copy of
their Results page for each test taken. If the testing provider goes down
again, then all tests taken during that time will be lost and cannot be
retrieved. If that happens then students will be asked to submit a copy of
each test's Results page as verification of completion of the test and their
grade on it. If the student does not have the results page then they will
have to retake the test in order to receive a grade for it.

OPEN BOOK FINAL TESTS by name:
(It is your responsibility to meet all requirements for each course before
retrieving the password for that course and taking any of the tests for it.)
(If the name in the following list is not underlined, then it is not yet an active
link and you must use the generic form to request your password.)
(When you use these direct links there is no wait time to receive the
password. It comes to you instantly when you submit the form.)
(If you do not receive the password instantly when you submit the form,
then it did not submit properly. If that happens then you must resubmit
the form.)
(You will receive two copies of the password- one instantaneously and an
autoresponse some time later. This is to ensure you receive the password.)
