WAIVER REQUEST FORM

This form is NOT to be used to request a waiver from the Administrative Fee.
If you desire a waiver from that fee, please click HERE.

Sometimes students need to request waivers from various courses, course work, or attendance requirements for various reasons, including physical limitations as well as Internet access limitations.  This is quite understandable.

If you desire a waiver, please use the Waiver Request Form to explain your request and the reasons for it.  We will then assess your request and notify you whether the waiver has or has not been granted.

Once you have submitted the form, you will be notified of our decision within 2 days; not including Saturday and Sunday when the SLBC offices are closed.  College offices are closed from 10:00 PM Friday night until 10:00 AM Monday morning.
